In order to secure your appointment, we require a $250 cosmetic consult fee. This fee is non-refundable and will be applied to any cosmetic procedure you choose to do in the office with Dr. Rokhsar for up to one year.

If you cancel your appointment at least 48 hours in advance, you may receive a full refund. If you cancel within 48 hours of your appointment, it may only be applied to a rescheduled appointment within one year.
